Dr. Basile, what is the right way to lose weight?

My method of slimming consists, therefore, in a real rehabilitation path to healthy nutrition in which, starting from a Mediterranean diet basis (UNESCO heritage nutritional model that leads to the strengthening of the immune system) various slimming steps are inserted to rapid impact, such as the ketogenic diet and intermittent fasting.

How is it possible to maintain the results achieved with so much sacrifice over time?

A fundamental rule is to never lose control of your body composition. For this reason, it is absolutely important to carry out periodic checks (at least 3 – 4 per year) by a qualified specialist, to constantly monitor the physical fitness achieved. Unfortunately, returning to unhealthy habits acquired over many years of life is by no means difficult. The secret is therefore to learn to respect the fundamental rules of “healthy eating” and make them your own, because a correct and balanced diet is necessary to protect our health (there are many diseases related to obesity and overweight: think of diseases cardiovascular, hypertension, dyslipidemia or psychological disorders). Obviously this does not mean making sacrifices for life because our body is absolutely able to amortize excess food in the long run! On the other hand, it is important to monitor the energy taken on average in a week (for which there will be more stringent days and other softer days in the so-called maintenance)! And again, when cooking, it is very important to accurately evaluate the portions, because in every family there is always “a weak link” that will end up consuming excess food!

In all these cases it is necessary to reopen the urethral canal and restore its functionality, all while preserving the sexual and aesthetic function.
In the presence of which symptoms it is advisable to consult a specialist?
Obviously the symptoms of urethral stenosis depend on the degree of stenosis itself: the most severe cases are characterized by gradually more severe disorders. Generally the patient complains: sensation of obstacle to the passage of urine, dripping shortly after urinating (post-voiding), incontinence, incomplete emptying of the bladder as well as the need to urinate often, burning when urinating, constant urinary infections sometimes even with fevers shaking, blood in urine and seminal fluid. How is this pathology treated today?
To resolve urethral stricture, surgery must be performed. The choice of surgery obviously depends on the characteristics of the pathology (cause, length and site) and the patient’s medical history (age, related diseases, general conditions, etc.).
Endoscopic urethrotomy is an absolutely minimally invasive technique, which consists of an endoscopic incision of the urethral canal using a “cold” blade (urethrotomy according to Sachse). The success of the method is around 60%: its main limitation is, in fact, represented by the fact that it can only be proposed in the case of short urethral strictures. The only valid long-term solution is represented by urethroplasty, a delicate specialized procedure that involves the removal and / or replacement of the stenotic urethral segment with an oral mucosal transplant. In expert hands, the success rate of this surgical technique is very encouraging (85%). In the case of a post-traumatic urethral stenosis of about 1 / 1.5 cm in length, the end to end anastomosis technique can be used, which guarantees a success of 92%.

Absolutely yes! Recall that this type of intervention has a triple implication: aesthetic, functional and psychological. The first is dictated by fashion, by the skill and aesthetic taste of the surgeon, by the physiognomy of a specific face. Nevertheless, the patient still presents himself today with photos taken from the internet or with photos of friends who have already undergone surgery and are used as models. An absurdity! The patient must be properly informed about the right aesthetic change to make his face perfect. Otherwise, the risk is to make a nose not suitable for his face or, even worse, to create noses that are all the same. The second aspect is no less important: the nose must be able to work! Modern society that has put appearance first, aesthetics above all else, has overshadowed the most important aspect of the nose, namely its respiratory function, which a good surgeon cannot neglect. In these years of activity I have divided the patients into two groups. All this to hide a defect that unfortunately is there in the center of the face and can be seen. If obsession arises, as soon as possible, they undergo surgery. These are the youngest patients, the most determined ones, with clearer ideas but also those who have higher expectations. After the surgery, they walk with their heads held high, take pictures of their faces with pride, face life more confidently, having strengthened their character thanks to a real psychological revolution. The second group is represented, however, by those people who have a defect, perceive it as such but are not obsessed with it. After the more delicate stages of growth, the patient arrives at adulthood with full self-acceptance until, for various reasons, he takes up an old idea put in the drawer, left there and embraces it again. Generally this type of patient wants small changes, which should not be noticed. This should not suggest that the aesthetic change will not affect the emotional sphere, on the contrary in this case the psychological implication is stronger. Dr. Nardi, what is the pathology of the diabetic foot?

The diabetic foot is one of the most serious complications of diabetes mellitus; a highly disabling pathology that develops as a consequence of diabetic neuropathy, an alteration of the peripheral nervous system that is manifested by tingling, cramps, gait disorders and alteration of the patient’s perceptive sensitivity (inability to perceive pain, hot and cold). Since in most cases diabetic neuropathy is also accompanied by an alteration of the circulation affecting the lower limbs (arteriopathy or vascular lesions), even a small wound could degenerate causing very serious damage, such as the formation of bleeding ulcers capable over time to evolve into gangrene (massive putrefaction and tissue necrosis that will have to undergo amputation). The timeliness of the diagnosis is therefore essential to avoid the progression of a disease that cannot be underestimated at all: the patient must necessarily keep the blood sugar level under control, following the prescribed visits, including that of the circulation of the lower limbs. If the vascular surgeon finds an arterial pathology, the same can be appropriately treated with drug therapy; in severe cases, peripheral angioplasty can be used, a minimally invasive interventional radiology procedure that is used to dilate an obstructed blood vessel, or with the bypass that improves blood circulation in the legs, preventing the disease from progressing inexorably.

What are the latest frontiers in the treatment of varicose veins, a jointly medical and aesthetic problem that afflicts many women?

The treatment of varicose veins can consist in closing the dilated vessel by injecting sclerosing substances in the form of liquid or foam, or in ablative endovascular techniques such as laser, radiofrequency, injection of particular glues, or even in the surgical removal of the vessel itself, obtained with different methods, all consisting of removing the venous segment through a small cut, all strictly under local or locoregional anesthesia and with the utmost respect for aesthetics. As in all medical branches, even in phlebology today there is a marked tendency to use increasingly minimally invasive, painless and repeatable techniques over time; methods increasingly patient-oriented due to ease of execution, effectiveness of treatment and extreme rarity of complications and side effects.

Dr. Nocera, can you tell us your story?

After earning a degree and a Masters in Management at the Luigi Bocconi University of Milan, I gained various experiences in prestigious consulting firms, both in Italy and abroad, and in particular in Madrid where I lived for about a year. The boundless love for my land, which gave birth to great artists and entrepreneurs, prompted me soon to return to my hometown, Naples, with a wealth of international experience that made me a visionary entrepreneur , forward-looking and open to the world. After winning the PhD in Economics and Public Administration Regulation, in 2011 I decided to found Nocera Consulting; a young and dynamic company – with offices in Naples, Casoria, Rome and Sofia (Bulgaria) – which in a short time affirmed its leadership in the field of Subsidized Finance, managing to increase its customer base thanks to a highly qualified consultancy service on the whole side of public and private financing to the company.

What is the company’s mission?

A premise appears necessary. In order to increase the competitiveness of the country system, to favor the development of new entrepreneurial activities and to support corporate investments in certain economic sectors, in recent years there has been a proliferation of various subsidized finance instruments and projects, whose sources derive from a rather articulated and complex ad hoc regulatory framework that you need to know perfectly. The loan covers 100% of the eligible expenses and consists of the non-refundable contribution equal to 35% of the total investment and the bank loan equal to 65% of the total investment, guaranteed by the Guarantee Fund for SMEs. However, the excessive bureaucratization especially of the reporting phase and nonetheless the regulatory complexity can represent an obstacle course for businesses and entrepreneurs; a path that can make access to these incentives difficult and incomprehensible, which instead requires the intervention of specialist and reliable skills, which is not easy to find on the market. Thanks to a competent multidisciplinary team, Nocera Consulting has quickly become the main point of reference in Southern Italy for this type of call. Even before the outbreak of this emergency, I was quite skeptical about the excessively welfare policy adopted by the government, which feeds unemployment and does not adequately reward entrepreneurs!
