Prof. Giancotti, what does Orthodontics do?

Today, three-dimensional digital radiology offers the possibility of making a rapid and precise diagnosis, with all the advantages that this entails: the 3D Cone Beam (CBCT), for example, provide images that faithfully reproduce the patient’s anatomy, allow you to observe the anatomical dimensions and to correctly evaluate the bone volume. This technique is therefore superior to the traditional one due to the greater definition of its images and because it allows better contrasts between structures of different densities, exposing the patient to a lower dose of radiation. 3D technology offers interesting perspectives also from a therapeutic point of view: in fact, in recent years there has been a shift from reactive devices in which it is not possible to accurately predict the effects but simply try to govern the reaction to others of a proactive type that foresee the effects of a specific clinical management procedure, allowing the specialist to plan increasingly targeted and customized therapies in time, minimizing unwanted effects. Ultimately, all modern orthodontic equipment should be based on proactive procedures.

Exactly! The use of digital technology arises from the need to work with greater safety and precision, reducing the duration of treatments and increasing the success rate of the work performed, with enormous benefits both in social and economic terms. In recent years, for example, there has been a growing interest in “invisible orthodontics”, a technique based on the use of progressive transparent aligners. Also in this case, thanks to the latest generation software, the specialist is able to create, with extreme precision, a virtual customized model as an initial basis for programming and simulate the smile at the end of the treatment, something much appreciated by patients. If it is then necessary to use fixed equipment, it is possible, in addition to traditional devices, to use robotic orthodontic wires that allow you to insert individualized information into the work span that generate extremely precise and faster tooth movements. Dr. Basile, what is the right way to lose weight?

A premise appears necessary. The main purpose of my work is to define a food plan aimed at improving the patient’s general health as well as his physical fitness, guiding him in the careful choice of quality and quantity of food. The scientific parameter that must be monitored to verify the effectiveness of the plan is the body composition that must be perfected in a precise and safe way. There is no right way to lose weight; On the other hand, there are specific dietary approaches, finely tested in the best universities in the world and endorsed by the scientific community, which obviously need to be personalized taking into account the psychophysical health of each individual. Personally, when the response of body and mind allows it, I often prefer to change my dietary approach so as to reach the set goal in the shortest possible time. Always in a safe and healthy way of course. Among other things, this ensures that people do not get bored following a certain food plan but are always motivated to reach the ideal weight. My method of slimming consists, therefore, in a real rehabilitation path to healthy nutrition in which, starting from a Mediterranean diet basis (UNESCO heritage nutritional model that leads to the strengthening of the immune system) various slimming steps are inserted to rapid impact, such as the ketogenic diet and intermittent fasting.

How is it possible to maintain the results achieved with so much sacrifice over time?

A fundamental rule is to never lose control of your body composition. For this reason, it is absolutely important to carry out periodic checks (at least 3 – 4 per year) by a qualified specialist, to constantly monitor the physical fitness achieved. Unfortunately, returning to unhealthy habits acquired over many years of life is by no means difficult. The secret is therefore to learn to respect the fundamental rules of “healthy eating” and make them your own, because a correct and balanced diet is necessary to protect our health (there are many diseases related to obesity and overweight: think of diseases cardiovascular, hypertension, dyslipidemia or psychological disorders). Obviously this does not mean making sacrifices for life because our body is absolutely able to amortize excess food in the long run! On the other hand, it is important to monitor the energy taken on average in a week (for which there will be more stringent days and other softer days in the so-called maintenance)! And again, when cooking, it is very important to accurately evaluate the portions, because in every family there is always “a weak link” that will end up consuming excess food!

Prof. Sansalone, what can be the causes of this highly disabling pathology?
The stenosis of the urethra is determined by the formation of scar tissue such as to cause partial or total obstruction; this formation may depend on various factors: urinary or sexual infections, trauma related, for example, to the use of urinary catheters or traffic accidents, diseases such as Lichen Sclerosus, tumors or unsuccessful hypospadias surgery. In all these cases it is necessary to reopen the urethral canal and restore its functionality, all while preserving the sexual and aesthetic function.
In the presence of which symptoms it is advisable to consult a specialist?
Obviously the symptoms of urethral stenosis depend on the degree of stenosis itself: the most severe cases are characterized by gradually more severe disorders. Generally the patient complains: sensation of obstacle to the passage of urine, dripping shortly after urinating (post-voiding), incontinence, incomplete emptying of the bladder as well as the need to urinate often, burning when urinating, constant urinary infections sometimes even with fevers shaking, blood in urine and seminal fluid. When the symptoms described are accompanied by pain and / or affect the standard of living (as in the case of acute urinary retention or renal failure), it is advisable to consult a specialist. In this type of surgery, in fact, it is necessary to rely on qualified centers, whose surgeons have numerous cases to their credit each year, successfully treated thanks to the experience of the operators themselves. Not surprisingly, the Sanatrix Clinic wanted to fill the lack of highly specialized structures in Central-Southern Italy, opening the Center for Urethral Reconstructive Surgery, which I directed, which in a short time became an important national and international point of reference for the diagnosis and treatment of urethral stricture: professionals with well over 3000 operations performed to date, with an average of 400 cases a year, 25% of which from all over the world!
How is this pathology treated today?
To resolve urethral stricture, surgery must be performed. The choice of surgery obviously depends on the characteristics of the pathology (cause, length and site) and the patient’s medical history (age, related diseases, general conditions, etc.).
Endoscopic urethrotomy is an absolutely minimally invasive technique, which consists of an endoscopic incision of the urethral canal using a “cold” blade (urethrotomy according to Sachse). The success of the method is around 60%: its main limitation is, in fact, represented by the fact that it can only be proposed in the case of short urethral strictures. The only valid long-term solution is represented by urethroplasty, a delicate specialized procedure that involves the removal and / or replacement of the stenotic urethral segment with an oral mucosal transplant. In expert hands, the success rate of this surgical technique is very encouraging (85%). In the case of a post-traumatic urethral stenosis of about 1 / 1.5 cm in length, the end to end anastomosis technique can be used, which guarantees a success of 92%.
