The doctors Cammarano and De Peppo illustrate the advantages of the minimally invasive front access technique

The evolution of surgical techniques, more and more accurate and safe than in the past, and the progress in the technology with which the orthopedic prostheses are made, more and more biocompatible and resistant to wear, make total hip arthroplasty one of the most success in orthopedic surgery. The tried and tested minimally invasive front access route is able to ensure excellent functional results for an ever-increasing number of patients.

Dr. Germano Cammarano, Director of the UOC Orthopedics General of the CTO in Rome and Dr. Marco De Peppo, Head of the Minimally Invasive Hip Surgery at the ARS Medica Hospital in Rome, spoke about this approach. It has been used routinely for 15 years, with excellent clinical results.

Dr. Cammarano, what are the particularities that make the minimally invasive anterior access particularly suitable for the implant of a hip replacement?

The front access is a well-known way, even as an extended access. The novelty of the minimally invasive pathway is that of using only one segment of this great surgical route characterized by an anatomical inter-nervous and inter-muscular interval.

Because of this peculiarity, this access path does not provide either detachments or muscle sections, immediately guarantees an optimal dynamic stability of the hip and allows a significant reduction of peri-operative blood loss, with less recourse to transfusions.

It also allows you to work with the patient in a supine position, considering the ideal position for anesthesia assistance, for the correct positioning of the implant with a thorough intraoperative control of the dysmetry.

Dr. De Peppo, the prosthetic surgery of the hip with a minimally invasive access to which patients offers the greatest advantages?

This technique offers great benefits to all patients. We use it both in the treatment of coxarthrosis and fractures of the neck and head of the femur, providing our patients with the best experience associated with the best clinical result, both in terms of improving the quality of life and the realization of increasingly stable implants. and better integrated with the bone and therefore longer lasting over time.

Thanks to the respect of nerve and muscular structures, the patient is able to start the fast-track rehabilitation process early, starting to walk the first postoperative day and making the stairs on the second day, allowing a quick return to normal daily activities, including sports, a goal that is much sought after by patients who are increasingly younger, demanding and willing to resume their lifestyle as soon as possible.
